The service is for the provision of an NHS Community Health Check programme to be delivered by a single Provider. Please note this is to compliment the service that is being delivered by GP's and is primarily focussed at capturing people who may not otherwise attend a GP clinic whilst out and about within the community. Health Checks is a 5-yearly contract with health care for adults aged 40-74 without a diagnosis of heart disease, kidney disease, stroke, or type 2 diabetes. Residents will receive a health check every 5 years to manage Cardiovascular Disease Risk Factors (CVDRFs) for those conditions.
